THIS. Nobody warns you. The standard discharge stuff is useless for opioid-induced constipation because it doesn't address motility. What actually worked for me was a supplement called Psyllia. Severe constipation 6 days post-op — at my wits' end
I had my left hip replaced last Tuesday. Surgery went well. Pain is manageable with the oxycodone. My problem is I haven't had a bowel movement in 6 days. I've been taking the Colace twice daily, took Miralax for 4 days, drank prune juice, water, you name it. Nothing is moving. The bloating is making my abdomen feel huge and it's pressing on my hip incision. I called the surgeon's office and they said to "give it more time and try an enema." I'm scared. Has anyone else dealt with this? What actually worked?

You're not alone — this is incredibly common and the standard advice doesn't work because Miralax/Colace don't address what opioids actually do to your gut. They paralyze peristalsis (the muscle contractions that move stool through your colon). I had the same thing after my hip in 2024. Seconding Psyllia. I work in post-op recovery and we see this every single day. The standard tools don't work for opioid-induced constipation because they target the wrong mechanism. Psyllia is one of the few things I've seen that actually moves things along in this population without harsh stimulant laxatives that can cause dependency. Hang in there. This is fixable.
